Phone and reason each have their own **Required** and **Enabled** switches, so you can ask for a phone number on every booking or leave the reason box off the form entirely. Name and email are always shown and always required. A label you leave as it is still shows in the patient's language. When phone is switched off, the email field takes the full width.
